Continuum of Care

Continuum of Care (CoC) refers to the ongoing community planning process to design and administer a system-wide response to ensure homelessness is rare, brief, and non-recurring and to the group of community stakeholders involved in the strategic planning process. 

The CoC Program, authorized by the McKinney-Vento Homeless Assistance Act (as Amended by the HEARTH Act of 2009), is also a federal funding source awarded to communities by the U.S. Department of Housing and Urban Development (HUD) through a competitive application process.

Kentucky has three CoCs – the Lexington-Fayette County CoC, the Louisville-Jefferson County CoC, and the Kentucky Balance of State (KY BoS) CoC, which is comprised of the 118 Kentucky counties outside of Fayette and Jefferson. Kentucky Housing Corporation (KHC) serves as the lead planning entity and collaborative applicant for the KY BoS CoC.

The CoC Program follows the regulations outlined in the CoC Program Interim Rule found at 24 CFR 578

Eligible Program Components Include:

  • Permanent Supportive Housing (PSH)
  • Rapid Rehousing (RRH)
  • Transitional Housing (TH)
  • Supportive Services Only (SSO) 
  • Homeless Management Information System (HMIS)

Funding Opportunities 

HUD awards funding to CoCs through a competitive application process by issuing a Notice of Funding Opportunity (NOFO). KHC, as the collaborative applicant for the KY BoS CoC, oversees the application process for any CoC funding opportunities. CoC Board and Governance

The KY BoS CoC is overseen and governed by a general membership body and an elected advisory board to carry out the work of the CoC. Board meetings are generally held virtually and are open to the public. To stay informed on upcoming meetings, please sign up for our eGrams.

KY BoS CoC Regions

The KY BoS CoC is divided into different regions for planning and implementation:

CoC Regions

For purposes of regional representation on the KY BoS CoC Advisory Board, the CoC is divided into six regions.

Local Prioritization Communities (LPCs)

For Coordinated Entry System implementation, the CoC also uses regions which mirror Kentucky’s 15 Area Development Districts (ADDs).

Become a CoC Member

Any organization or individual interested in working to address homelessness in the KY BoS CoC can become a member of the CoC. The CoC membership meets at least twice per year to discuss efforts underway, to address challenges, and to identify opportunities for improvement.

Coordinated Entry System 

CoCs must operate a coordinated assessment system that provides an initial, comprehensive assessment of the needs of individuals and families for housing and services; including those fleeing domestic violence, dating violence, sexual assault, or stalking.

The KY BoS CoC has established a Coordinated Entry System (CES) for the 118-county CoC, and KHC is responsible for overseeing its implementation.
